Membership Documents

Waiver / liability release

The Lions Den is committed to providing a positive, safe, disciplined, faith-centered, and family-oriented environment for all participants.

Participation in any program offered by The Lions Den is a privilege. All participants, parents, guardians, family members, coaches, volunteers, and spectators are expected to conduct themselves in a manner that reflects integrity, respect, accountability, sportsmanship, and good character.

PARENT, GUARDIAN, FAMILY MEMBER, AND SPECTATOR EXPECTATIONS

I agree to:

• Promote the values of faith, integrity, discipline, accountability, respect, and good character.

• Encourage positive sportsmanship and demonstrate respect toward athletes, coaches, instructors, staff members, volunteers, officials, competitors, and spectators.

• Support the emotional, physical, and mental well-being of all participants.

• Arrive on time and prepared for practices, classes, competitions, lessons, camps, clinics, and events.

• Ensure participants maintain proper hygiene, including trimmed fingernails and toenails and clean training equipment.

• Ensure participants wear appropriate attire and follow all equipment requirements established by The Lions Den.

• Conduct regular checks for skin conditions, illnesses, or injuries and comply with all health and communicable disease policies.

• Immediately notify coaches or staff of any injury, illness, medical concern, or condition that could affect participation.

• Refrain from entering training areas unless authorized by staff or in the event of an emergency.

• Allow coaches and instructors to coach without interference.

• Refrain from "sideline coaching" during classes, practices, competitions, or events.

• Address concerns respectfully through appropriate communication with staff, coaches, or management.

• Refrain from gossip, harassment, intimidation, bullying, threats, discrimination, profanity, abusive language, or disruptive conduct.

• Treat all individuals with dignity and respect regardless of age, race, religion, gender, ability level, background, or experience.

• Support and uphold The Lions Den Anti-Bullying Program and maintain a bully-free environment.

PARTICIPANT EXPECTATIONS

I agree to:

• Demonstrate respect toward coaches, instructors, staff, teammates, training partners, competitors, officials, volunteers, and spectators.

• Follow all rules, safety procedures, instructions, and policies established by The Lions Den.

• Practice good sportsmanship at all times.

• Refrain from bullying, harassment, intimidation, threats, fighting, abusive conduct, or disruptive behavior.

• Use appropriate language and behavior at all times.

• Respect training partners and prioritize safety during all activities.

• Arrive prepared, attentive, and ready to learn.

• Report injuries, illnesses, skin conditions, or safety concerns immediately.

• Maintain proper hygiene and cleanliness standards.

• Represent The Lions Den with integrity both inside and outside the facility.

DISCIPLINE AND PARTICIPATION

The Lions Den reserves the right to suspend, remove, restrict, or terminate participation privileges for any participant, parent, guardian, family member, or spectator whose conduct is deemed unsafe, disruptive, inappropriate, threatening, abusive, dishonest, or contrary to the mission and values of The Lions Den.

Consequences may include verbal warnings, temporary suspension, removal from individual classes or events, membership suspension, membership termination, or permanent removal from participation.

Any disciplinary decision made by The Lions Den shall be final.

No refunds, credits, prorated tuition, or membership adjustments shall be issued as a result of disciplinary action, suspension, or removal.

PHOTO AND VIDEO RELEASE

I grant The Lions Den permission to photograph, videotape, livestream, record, and otherwise capture images, likenesses, voices, and participation of myself and/or my minor child during classes, practices, competitions, camps, clinics, special events, and activities.

I authorize The Lions Den to use such photographs, videos, recordings, and images for promotional, educational, fundraising, advertising, social media, website, marketing, print, and other lawful business purposes without compensation.

I understand that The Lions Den cannot control or prevent the sharing, downloading, reposting, or redistribution of photographs or videos by third parties once they are publicly released.

I release and hold harmless The Lions Den from any claims arising from the authorized use of photographs, videos, recordings, or media content.

ACKNOWLEDGMENT

By signing electronically, I acknowledge that I have read, understand, and agree to comply with this Parent, Participant, and Family Code of Conduct and Photo and Video Release.

If signing on behalf of a minor participant, I certify that I am the parent or legal guardian authorized to enter into this Agreement on behalf of the minor.

THE LIONS DEN

LIABILITY WAIVER, ASSUMPTION OF RISK, MEDICAL AUTHORIZATION, AND RELEASE OF LIABILITY AGREEMENT

PLEASE READ CAREFULLY. THIS DOCUMENT AFFECTS YOUR LEGAL RIGHTS.

In consideration of being permitted to participate in any class, lesson, camp, clinic, tournament, competition, open mat, anti-bullying program, fitness activity, strength and conditioning activity, private lesson, special event, or any other activity offered by The Lions Den ("Activities"), I agree as follows:

  1. ASSUMPTION OF RISK

I understand that participation in wrestling, Brazilian Jiu-Jitsu, martial arts, anti-bullying programs, fitness activities, strength and conditioning programs, competitions, camps, clinics, tournaments, and related activities involves inherent risks that cannot be eliminated regardless of the level of supervision, coaching, instruction, or safety precautions provided.

These risks include but are not limited to:

• Cuts, bruises, scrapes, and abrasions

• Sprains and strains

• Fractures and broken bones

• Dislocations

• Concussions and head injuries

• Neck, spinal, and back injuries

• Permanent disability

• Paralysis

• Exposure to communicable diseases and skin infections

• Serious bodily injury

• Death

I knowingly and voluntarily assume all risks associated with participation in Activities offered by The Lions Den, whether known or unknown, foreseeable or unforeseeable.

  1. HEALTH REPRESENTATION

I certify that I, or my minor child, am physically capable of safely participating in Activities offered by The Lions Den.

I understand that participation should not occur if I or my child have a medical condition that may place us or others at risk.

I agree to notify The Lions Den of any medical condition, injury, illness, limitation, allergy, or concern that may affect participation.

  1. RELEASE OF LIABILITY

To the fullest extent permitted by law, I release, waive, discharge, and covenant not to sue The Lions Den, its owners, directors, officers, board members, coaches, instructors, employees, volunteers, independent contractors, sponsors, affiliates, facility owners, landlords, and representatives (collectively referred to as the "Released Parties") from any and all claims, demands, causes of action, damages, losses, liabilities, costs, expenses, or injuries arising from or related to participation in any Activities.

This release includes claims arising from the ordinary negligence of the Released Parties.

  1. INDEMNIFICATION

I agree to indemnify, defend, and hold harmless the Released Parties from any claims, demands, lawsuits, attorney fees, costs, damages, liabilities, or expenses arising from my participation or my minor child's participation in Activities offered by The Lions Den.

  1. COMMUNICABLE DISEASES AND SKIN CONDITIONS

I acknowledge that participation in wrestling, Brazilian Jiu-Jitsu, martial arts, and other close-contact activities may expose participants to communicable diseases, skin infections, bacterial infections, viral infections, fungal infections, and other contagious conditions.

I understand that The Lions Den cannot guarantee a disease-free environment.

I agree to comply with all health, hygiene, skin condition, and communicable disease policies established by The Lions Den, including any requirements for medical evaluation, treatment, or clearance before returning to participation.

I further acknowledge that I have received, reviewed, and agree to comply with The Lions Den Skin Infections and Communicable Conditions Policy.

  1. MEDICAL AUTHORIZATION

In the event of an injury, illness, accident, or medical emergency, I authorize The Lions Den and its representatives to obtain emergency medical care for myself or my minor child when a parent, guardian, or emergency contact cannot be reached immediately.

I understand that all medical expenses remain my sole responsibility.

  1. RULES, DISCIPLINE, AND SAFETY

I agree to comply with all rules, policies, safety requirements, coach instructions, participation standards, and facility guidelines established by The Lions Den.

I understand that coaches and staff may conduct visual skin checks or other reasonable safety inspections when necessary to protect participants.

The Lions Den reserves the right to remove, suspend, or terminate participation for unsafe behavior, disruptive conduct, bullying, harassment, threats, misconduct, failure to follow instructions, or violations of club policies.

Removal from participation does not entitle any participant to a refund.

  1. PROPERTY DAMAGE AND PERSONAL BELONGINGS

I understand that participation may result in loss, theft, or damage to personal property.

The Lions Den is not responsible for lost, stolen, or damaged personal property.

  1. PHOTO AND VIDEO RELEASE

I grant The Lions Den permission to photograph, videotape, record, and use my or my child's image, likeness, voice, and participation for promotional, educational, social media, website, advertising, marketing, fundraising, and other lawful business purposes without compensation.

  1. SEVERABILITY

If any provision of this Agreement is determined to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

  1. GOVERNING LAW

This Agreement shall be governed by and interpreted under the laws of the Commonwealth of Pennsylvania.

  1. ELECTRONIC SIGNATURE

I understand that my electronic signature shall have the same legal force and effect as a handwritten signature.

  1. ACKNOWLEDGMENT OF UNDERSTANDING

I acknowledge that:

• I have carefully read this Agreement in its entirety.

• I fully understand its contents.

• I understand that I am giving up substantial legal rights.

• I understand the risks associated with participation.

• I voluntarily agree to all terms contained in this Agreement.

PARENT/GUARDIAN CONSENT FOR MINORS

If the participant is under eighteen (18) years of age, the parent or legal guardian signing electronically acknowledges that they have read, understood, and agreed to all terms of this Agreement on behalf of the minor participant and further agree to be bound by all provisions contained herein.

By signing electronically, the participant and/or parent or legal guardian acknowledges that they have carefully read this Agreement, understand its contents, understand that substantial legal rights are being waived, and voluntarily agree to all terms and conditions contained herein.

SKIN INFECTIONS AND COMMUNICABLE CONDITIONS POLICY

The Lions Den is committed to providing a safe training environment for all participants.

Parents and athletes acknowledge that wrestling, martial arts, and other close-contact sports may increase the risk of exposure to contagious skin conditions and communicable diseases, including but not limited to ringworm, impetigo, staph infections, MRSA, herpes gladiatorum, molluscum contagiosum, and other bacterial, viral, or fungal infections.

Parents and athletes agree to inspect for potential skin conditions regularly and immediately report any suspected contagious condition to The Lions Den.

If a coach, instructor, staff member, volunteer, or representative of The Lions Den observes a skin condition, lesion, rash, wound, infection, or other condition that may pose a health risk to participants, The Lions Den reserves the right to immediately remove the athlete from participation.

The athlete may not return to practices, classes, competitions, camps, clinics, or other activities until:

• A licensed medical provider has evaluated the condition; and

• Written medical clearance is provided to The Lions Den; and

• The Lions Den determines that participation may resume safely.

The Lions Den reserves the right to require additional medical documentation when deemed necessary to protect the health and safety of participants.

Any decision regarding participation restrictions made by The Lions Den for health and safety reasons shall be final.

No refunds, credits, prorated tuition, or membership adjustments will be provided for absences resulting from illness, injury, contagious conditions, medical restrictions, or failure to obtain required medical clearance.

Coaches and staff of The Lions Den may perform visual skin checks when necessary to protect the health and safety of participants.

Membership Agreement & Billing Authorization

This Membership Agreement ("Agreement") is entered into between The Lion's Den ("The Lion's Den") and the member and/or parent or legal guardian of a minor participant.

By enrolling in any membership, class, program, camp, clinic, competition, or activity offered by The Lion's Den, the member and/or parent or legal guardian acknowledges that they have read, understand, and agree to all terms contained in this Agreement.


Membership Enrollment

Membership provides access to the programs, classes, training sessions, camps, clinics, competitions, and activities associated with the selected membership, subject to all rules, policies, procedures, schedules, and membership requirements established by The Lion's Den.

Unless otherwise specified as a seasonal or limited-term membership, memberships automatically renew on a month-to-month basis until properly canceled in accordance with this Agreement.

Memberships are non-transferable.


Billing Authorization

By electronically signing this Agreement, the member and/or parent or legal guardian authorizes The Lion's Den to automatically charge the payment method provided for all authorized charges associated with participation, including but not limited to:

  • Membership fees
  • Registration fees
  • Seasonal program fees
  • Camps and clinics
  • Private lessons
  • Tournament and event fees
  • Merchandise purchases
  • Travel expenses
  • Outstanding account balances
  • Other authorized charges associated with participation

Recurring membership payments will automatically process on the scheduled billing date.

The member and/or parent or legal guardian is responsible for maintaining a valid payment method on file at all times.

The Lion's Den reserves the right to modify membership rates, fees, class schedules, program offerings, and club policies upon reasonable advance notice to members. Continued participation after the effective date of any announced change constitutes acceptance of the revised rates, fees, schedules, programs, or policies.


Registration Fee

A non-refundable $25.00 registration fee is required each time a new membership is started.

If a membership is canceled and later restarted, a new $25.00 registration fee will apply.


Payment Policies

Membership fees are due on the scheduled billing date regardless of attendance.

No refunds, credits, or prorated adjustments will be issued for missed classes, vacations, holidays, schedule conflicts, injuries, illnesses, suspensions, voluntary non-attendance, or unused training time unless otherwise required by applicable law.

The member is responsible for maintaining a valid payment method on file at all times.

If any scheduled payment is declined, returned, or otherwise fails to process for any reason, a $25.00 processing fee will be assessed for each failed payment that requires reprocessing.

Returned payments, chargebacks, declined transactions, or failed payment attempts may also be subject to collection efforts.

The Lion's Den reserves the right to suspend membership privileges, including participation in classes, practices, camps, clinics, tournaments, private lessons, and other club activities until all outstanding balances and applicable fees have been paid in full.

All membership fees, registration fees, program fees, camp fees, clinic fees, tournament fees, merchandise purchases, and all other payments made to The Lion's Den are non-refundable.


Cancellation Policy

Membership cancellations must be submitted in writing by email to The Lion's Den Billing Department at the designated billing email address provided by The Lion's Den.

Cancellation requests submitted verbally, by telephone, text message, Facebook message, Instagram message, social media message, mobile application message, or by notifying a coach, instructor, volunteer, front desk employee, or any other staff member will not be accepted and will not constitute official notice of cancellation.

The Lion's Den requires a minimum of thirty (30) days' written notice before a membership may be canceled.

The thirty (30) day cancellation period begins only when the cancellation email is received by The Lion's Den Billing Department.

If a scheduled payment falls within the required thirty (30) day notice period, that payment remains due and payable and will be processed as scheduled.

Membership remains active during the required notice period, and members may continue participating during that time.

Membership fees charged during the required notice period are non-refundable.

Seasonal memberships, prepaid programs, camps, clinics, tournaments, and special events are governed by the terms of the specific program and are generally non-refundable unless otherwise stated in writing by The Lion's Den or required by applicable law.


Membership Freeze Policy

Each membership is permitted one (1) membership freeze per calendar year.

Membership freezes are limited to a maximum of thirty (30) consecutive days.

Freeze requests must be submitted in writing and approved by The Lion's Den before the requested freeze period begins.

At the conclusion of the approved freeze period, membership billing will automatically resume using the payment method on file.

A membership freeze does not constitute cancellation. Members wishing to cancel during or after a freeze must still submit an email to The Lion's Den Billing Department and comply with the full thirty (30) day cancellation policy.

Additional freezes may be approved solely at the discretion of The Lion's Den.


USA Wrestling Membership Requirement

Every athlete participating in any wrestling-related activity at The Lion's Den must maintain a current and valid USA Wrestling Membership.

A valid USA Wrestling Membership must be obtained before stepping onto the wrestling mat for any wrestling practice, clinic, camp, tournament, private lesson, open mat, or any other wrestling-related activity.

Members and parents are responsible for maintaining an active USA Wrestling Membership and providing accurate membership information to The Lion's Den upon request.

Athletes without proof of a current USA Wrestling Membership will not be permitted to participate in wrestling activities until valid proof has been provided.


Electronic Communication Authorization

By enrolling with The Lion's Den, the member and/or parent or legal guardian authorizes The Lion's Den to communicate regarding membership, billing, schedules, weather cancellations, emergency notifications, practices, tournaments, camps, clinics, events, announcements, and other club-related business through:

  • Email
  • Text Messages (SMS)
  • Telephone
  • Gymdesk Notifications
  • Mobile Applications
  • Electronic Documents
  • Electronic Signature Platforms
  • Other electronic communication systems utilized by The Lion's Den

The member and/or parent or legal guardian is responsible for maintaining accurate and current contact information, including email addresses, telephone numbers, mailing addresses, and emergency contacts.

The Lion's Den is not responsible for missed communications resulting from outdated, incorrect, blocked, or unavailable contact information.

Consent to receive promotional electronic communications may be withdrawn where required by applicable law; however, withdrawal of promotional communications does not prevent The Lion's Den from sending necessary billing, scheduling, emergency, safety, membership, or other transactional communications.


Member Responsibilities

Members and parents agree to comply with all rules, policies, procedures, facility requirements, schedules, coaching instructions, safety requirements, participation standards, and Codes of Conduct established by The Lion's Den.

Failure to comply with club policies may result in suspension or termination of membership without refund.

Members are responsible for keeping all enrollment information accurate and current.


Governing Policies

Members acknowledge that separate documents form part of the enrollment and participation process, including but not limited to:

  • Liability Waiver & Release
  • Parent & Athlete Code of Conduct
  • Skin Infection & Communicable Condition Policy
  • Travel, Transportation & Event Authorization
  • Media Release
  • Athlete Safety Policies
  • Facility Rules
  • Tournament Rules

These documents are incorporated by reference into this Agreement.


Right to Refuse, Suspend, or Terminate Membership

The Lion's Den reserves the right to refuse, suspend, or terminate any membership for reasons including, but not limited to:

  • Non-payment
  • Repeated declined payments
  • Failure to comply with club policies
  • Unsafe conduct
  • Harassment
  • Threatening or abusive behavior
  • Conduct detrimental to the safety, mission, reputation, or operation of The Lion's Den

Suspension or termination under this section does not entitle the member to a refund unless otherwise required by law.


Governing Law & Severability

This Agreement shall be governed by the laws of the Commonwealth of Pennsylvania.

If any provision of this Agreement is determined to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.


Agreement

By electronically signing below, the member and/or parent or legal guardian acknowledges that they have carefully read, understand, and voluntarily agree to all terms contained in this Membership Agreement & Billing Authorization.

The member and/or parent or legal guardian authorizes recurring and other authorized payments in accordance with the selected membership and agrees to comply with all current and future policies established by The Lion's Den.

The electronic signature provided through The Lion's Den enrollment system shall have the same legal force and effect as an original handwritten signature.

TRAVEL, TRANSPORTATION & SPECIAL EVENTS AUTHORIZATION

The Lions Den occasionally participates in tournaments, camps, clinics, competitions, overnight events, team travel, and other special activities that may require transportation, lodging, supervision, and participation away from the regular training facility.

By signing this authorization, the participant and/or parent/legal guardian acknowledges and agrees to the following:

  1. TRAVEL AUTHORIZATION

I authorize The Lions Den, its coaches, instructors, staff, volunteers, designated drivers, and representatives to transport the participant to and from approved events, practices, camps, clinics, tournaments, competitions, hotels, restaurants, training facilities, and other event-related locations.

Transportation may occur by personal vehicle, rental vehicle, bus, van, rideshare service, commercial transportation, or other reasonable means deemed appropriate by The Lions Den.

  1. ASSUMPTION OF TRAVEL RISKS

I understand that travel involves inherent risks including, but not limited to:

• Automobile accidents
• Bus or van accidents
• Weather-related incidents
• Hotel and lodging risks
• Injuries during travel
• Injuries during competition or training
• Illness or medical emergencies
• Acts of other participants or third parties

I voluntarily assume all risks associated with participation in travel and special events.

  1. EMERGENCY MEDICAL AUTHORIZATION

If I cannot be reached in an emergency, I authorize The Lions Den and its representatives to obtain emergency medical evaluation, treatment, transportation, hospitalization, and care deemed necessary for the participant.

I understand that reasonable efforts will be made to contact the parent or guardian before treatment whenever possible.

I accept full responsibility for any medical expenses incurred.

  1. HOTEL, CAMP, AND OVERNIGHT EVENTS

I acknowledge that overnight camps, tournaments, clinics, and travel events may involve hotel stays, dormitories, shared lodging, group supervision, and activities outside normal training hours.

I understand that The Lions Den cannot provide continuous one-on-one supervision and that participants are expected to follow all rules, instructions, and safety requirements.

  1. ATHLETE CONDUCT

Participants are expected to conduct themselves in a respectful and responsible manner at all times.

Failure to follow team rules, coach instructions, hotel policies, event rules, or applicable laws may result in disciplinary action, removal from activities, transportation home at the parent's expense, suspension, or termination from The Lions Den programs.

  1. FINANCIAL RESPONSIBILITY

The participant and/or parent/guardian remains responsible for all travel-related costs, including but not limited to tournament fees, registration fees, transportation costs, lodging expenses, meals, admissions, medical expenses, and other associated charges unless otherwise stated in writing by The Lions Den.

  1. RELEASE OF LIABILITY

To the fullest extent permitted by law, I release, waive, discharge, and hold harmless The Lions Den, its owners, officers, directors, coaches, instructors, employees, volunteers, representatives, and affiliates from any and all claims, liabilities, damages, losses, or expenses arising out of participation in travel activities, transportation, overnight events, camps, clinics, competitions, or related activities, except where prohibited by law.

  1. PHOTO AND MEDIA CONSENT

I acknowledge that photographs, videos, livestreams, interviews, and other media may be captured during travel events, camps, competitions, and special activities and may be used by The Lions Den for promotional, educational, social media, website, and marketing purposes without compensation.

  1. ACKNOWLEDGMENT

I have read this Travel, Transportation & Special Events Authorization, understand its contents, and voluntarily agree to all terms contained herein.
